COURSE SUBMISSION

All courses must be reviewed every 6 years. Reviews must be completed by Cluster Technical Review Committees (CTRC). Course work and forms are completed in the Student Information System (SIS).

  • FORMS THAT MUST BE COMPLETED IN SIS
Forms that NO LONGER NEED TO BE FILLED OUT

As long as the date that the CTRC was held is entered into the Curriculum Course Submission Form these two forms are no longer needed.

    • Cluster Technical Review Committee (CTRC) form
    • Changes to Existing Course

Forms that must be filled out:

    • Environmental Health and Safety
This form is required only if the course includes 1. chemicals 2. blood or 3. dangerous machinery

    Courses that seek GE:

UC Courses must complete all three forms, CSU only need the two applicable forms of these forms before it can go to Articulation for GE Review:

    Other:
    • Distance Education
This is handled through Canvas, I’ll enter this in after faculty complete the necessary forms in Canvas.
Only once all required items are completed (CTRC, SIS draft/forms) fill out this form so that courses can go to CRC.
